Hours: Every Thursday | 9:00 AM – 4:30 PM
What to bring (if available):
- Proof of address (dated within the last 6 months)
- Proof of income
- Proof of immigration or citizenship status
- Medicare card (if applicable)
- Proof of current resources (for long-term care applications)
